In order to best serve the client we have compiled a list of questions case managers can ask to learn more about a client. These questions can help case managers identify the service needs of clients in order to either connect them to services provided internally within the organization or refer them to outside organizations that provide the service. The answers to these questions can then be scored in order to determine the level of case management the client needs.
